Help desperately needed - Canadian work permit

I'm very close to completing my application for a work permit. All I have left to upload is my IMM1295 work permit application form but I can't download the form despite the fact that I have the latest version of Adobe. Could someone please e-mail me the correct form. I have very little time left before my acceptance…
