FIS renewal form

    My fis was stopped as I never got the renewal form as they had not updated my address. I was advised to send a letter with my address and request that they send me out a renewal form. that was two weeks ago and I have not got the form yet.
    Does anyone know if the renewal form is different from the original form? Would I have to start as if a new applicant if I just got that form and sent it? I tried to call but got no answer and then my credit ran out.

    The renewal form is different, there are only two pages, it's pretty much to confirm your details and make sure they haven't changed.
