You can get the form online, but it has to be filled in, signed and sent to your local council http://www.checktheregister.ie/PublicPages/AppForms.aspx

If you aren't on the register and want to vote in the local/European elections you need to apply for the supplemental register before May 6th. This will involve getting a guard to sign the form so don't leave it to the last minute.
