I am looking to do a claim for tax year 2012. Im using online calculator to see if i am due anything. For my gross pay should i put in the amount in column A? This is the Total Pay less pension contributions which i make through payroll? Or should i use Column C? This is total pay for USC purposes?

Its column A (on the 2013 P60s it might say on the left hand side, half way down the form, "use this figure in PAYE Anytime" but I dont know if thats in the 2012 ones).

If i was to request a P21 Balancing statement for 2012 through the Revenue Login site would it give me a refund for year 2012? My calculations seem to me that i overpaid by approx 2k.

Yes, request a P21 & Revenue will calculate yr tax (you can go back 4 years) - include any medical expenses too if you have them.
