USC AND MEDICAL CARD

Hi I was unemployed in 2012 and received a medical card I was lucky to get a job at end of 2012.
As I had a medical card I got to pay the reduced rate of 4% rather than the full 7%.
I lost my medical card in November but was wondering do I have to tell revenue or employer to move back to full rate of 7% for 2014 or will this happen automatically?

It expired hadn't looked at expiry date when I got it,tried to apply on-line and was refused, although I heard and read somewhere once you were unemployed you were entitled to it for 3 years but this may have changed.
