    We all know that a sole trader trading under his own name does not need to register a business name...

    e.g. John Murphy.

    and a sole trader trading as another name

    e.g. Acme Plumbers

    should.

    But in reality there are loads of sole traders who didn't take the time to register their business name.

    The classic example being a DJ who might trade as "xyz entertainments"
    or a plumber called "town_name plumbing service" (these are both fictional names)

    Does the CRO ever do anything about this?
    It would be in it's interest because it charges EUR 20 per registration,
    although the effort of sending letters might cost more than that?

    It's a good point.

    The fee is €40 (€20 online) so it should be worth their while to write the letter.

    Why not email them to ask them?

    They're not in a position to do this unless it somehow comes to their attention that you are carrying on business under an unregistered name. And why would it?
