Help with a getting a new log book

    Hey all, really need help with getting a form to get a new log book for a 05 car. I live in Kerry, and I've been in to the tax office twice now to check if there were any forms that I could go to the gardai with in order for me to send off for it. I asked for a form and was told come back another time and they'll be available... which I did. Returned another day and same thing happened again...and I am now very frustrated. I also went to the local gardai and of course they told me go back to the tax office...the joys. I've looked online for a copy of the form but I can't find anything. I want to sell my car ASAP and this is really annoying me.

    Can anyone help me out? Is there a copy of the form online?? I would be beyond grateful for any help, thank you!! :)
