what does a 'long form' birth cert look like anyway?

i need one of these to get a passport but i'm not sure if i have the right sort. the one i have says "form A2" in the top right corner. can someone post a link to a picture of the long form please?

Not any more.

"New" long form birth certs are an A4 printed sheet.

Looks like a short form birth cert but longer.

Why is there two versions of the birth cert?one of which is has a higher status than the other for some reason.

The long version has to have your parents' names on it for the purposes of passport application - that's the main difference, as far as I know.
