Crash cause

I had a crash yesterday while accelerating explosively in the small ring, What happened I believe is that the chain dropped of the small ring to the inside and the out of saddle swaying I couldn't recover it and wiped out. Is this a likely scenario?
And any idea what caused the chain to drop? FD misaligned? Worn chain/chainrings?
Thankfully nothing broken just a few cuts and bruises.
Is it common for a chain to drop in this way?

While it shouldn't happen if the front DR limit screws are set-up correctly, it can and has to me occasionally. You can put on chain catcher to limit the effects of this, though I've never tried myself.
