Passed Theory Test - Now What

    I have my:
    A completed application form ( D201,PDF)

    Two passport-type photographs (signed on back)

    The appropriate fee

    First Time doing theory test so dont need to bring learner permit

    An original birth certificate/passport

    A completed medical report D501 form (if your are downloading the Medical Report form you must print it back to back)
    IS THIS A MUST BECAUSE I HAVE NO PROBLEMS OR NOR I TAKE ANY MEDICAL PRESCRIPTIONS

    A completed eyesight D502 form

    Theory test certificate


    Do i send these and post it to my local tax office?
    Or do i have to drive there?

    You only need a medical report if you have a condition that may affect your driving.

    No you do not need to go into the tax office, you can post it to them.

    Yes, post it. It should be back within 2 weeks at the maximum, mine was much less.
