Equal Opportunities

I've been looking for a job for a good few years now and this has just occurred to me.
When applying for a job online, at the very end of the process before you get to submit your application, there is nearly always an "equal opportunities" questionnaire that you can either opt to fill in or leave blank.

My question is, is it better to fill it out or to leave it blank?
Does the information provided in this questionnaire (sex, religion, race, age and dissability) really have no effect on the decision made by that particular business?

Seems to me like a catch 22 situation. If you fill it out, the employer can make a decision based on that information but if you don't fill it out they can choose to not process your application based on that decision.

When I used to open these (before email). They would be separated with a tracking number. The shortlisting or interview panel would never ever see the equal opps questionnaire.

I ask myself the same question everyday!
I'm not sure why. I have 3 years bar experience and still never get called for so much as an interview. I'm only looking for a student-y job. Nothing professional.
