I bought Nexus Media Importer app for my Nexus 7. I now have a Nexus 4. Can I use this app on my nexus 4, or do I have to pay for it again and download it to my Nexus 4?

Its per account not device, once they're both on the same google account you pay once for your devices.

If you're going to use the Nexus Media Importer to read USB pen drives or HDDs I don't think it'll work as I don't think the Nexus 4 is capible of reading external USB drives (the 10 and 7 will), something about a hardware block or some such vs the two tablets that just need that app.
