When you're discharged the doctor will do your prescription, GP letter etc. Remind them that you need a cert for social welfare and they will give it to you. Ask the nurse looking after you and they will remind the doctor.

Ring the social welfare office, they'll send the officer around with some hot chicken soup to help you recover.

Oh and you should make your font colour Red to be sure everyone sees it.
And size 14.
And Bold.
Underline it.
