gears not working well

Using a ultegra rear derailleur and ultegra sti shifters but had a weird thing happen last week. Lost all tension on the rear derailleur cage and chain clattered till I stopped pedaling (smartishly I might say) Gave the cage a waggle and worked again to a fashion! Got home, took derailleur off and gave it a good clean. Back on I just can't get middle cogs to tune properly. Tried replacing cable, adjusting tension etc. and the cassette and chain are two weeks old. Any suggestions as to what could be wrong. Bent cage perhaps? Low and high gears seem ok and the noise of chain not tuned is more apparent when I put more pressure on. When I say tuned I mean the chain doesn't sound completely in gear but changing tension doesn't help!

    Help!

Bit of a simple suggestion but have you tried Tuning your low/high limit screws on the rear derailleur?

Yep, tried that too so the high /low are ok

Did you play with the barrel adjusters to fine tune in the gears you're worried about?

    they/them/theirs

    What caused the initial fault?

yep, tried all the fine tuning and as to how it happened was cycling along fine and dropped into inner ring to pull away from lights and going up a slight rise the chain made a big grind and looking down the cage was not pulling back, tensioning chain, hence the chain not meshing on cassette and dirty big clatter! Pulled over and gave the cage a few backward forward flexes which seemed to free it but wasn't great. Had to use big ring all way home. Cleaned the derailleur thoroughly and seems to be ok tension wise but as stated, just cannot get middle rings to tune hence reckon the cage is bent. Been trying to find reference as to how the cage should look as have been having a good look at the cage and must admit it doesn't look straight! Outer cage looks warped and the gap between the two cages is not constant, getting much wider at top making it look warped. Shall be going to LBS to have a gander at a new derailleur to compare.

Sounds like you need a new rear derailler so.

It could be chain suck that caused it. Follow this link for some pointers.

If it was chain suck I wouldnt be having tuning problem which I still am with a new derailleur! Possibly hanger is bent then. Trip to LBS I think.
