Form UP 14b - number of pages?

    Hi all

    Bit of background: I've been on JSA for a while & am just after getting a part-time job (week on/week off). I called into my local social welfare office today to enquire about claiming for the weeks I won't be working, and they gave me some forms to take away - UP 14a for me to fill in, UP 14b for my employer to fill in, and UP16 for both of us to fill in.

    Now, the question: from what I've been given, UP 14a is clearly a two-page form and UP16 is clearly a one-page form. However, the UP 14b I've got is a single photocopied page - it has a Part 1 and then just stops, without a signed declaration space like the others.

    Does anyone know if this is correct, or if it should be longer? I can't find anywhere online to check, and there isn't a copy available to download on welfare.ie.

    That form is 2 sided. Usually 1 page front and back unless as in your case it was photocopied and they didnt attach the second page. Ring the office ask them to send you a new form.

    That's what I thought. Thanks for confirming it.

    Guess I'll be calling on them again tomorrow. It's ridiculous that all of the forms aren't available online, especially when the offices can't be bothered to check whether they're giving the proper ones.
