I lost my log book while moving house.
Do i need a change of address form separate to the rf134 replacment documents form?

Usually you get the rf134, then get the new cert, then do change of address on the back of the new cert.

Cert will be posted to your old address after you apply for it in the tax office.

If you enclose a covering note with the new address on the form it will be sent to your new address.
It's to be sent to DOE in Shannon.

You don't send the RF134 to the DOE.

You bring it to the motor tax office, pay for the replacement cert, then they issue you a receipt. It's then printed and sent from Shannon.

Maybe you can request it to be sent to a different address, If so, that's a serious flaw in the system where I can pretend to be the owner of any car I see parked outside someones house, once I know their name, I can go to the Gardai and get RF134 stamped, go to tax office, ask them to send it to a different address and I now have a copy of the reg cert without the actual owner knowing a thing.

Are you taxing the car? If so you can apply for pin online to be sent to your email and while retaxing the car online there is the facility to change address.
