Vaccinations for SLT students

I can not find the list of all the vaccinations needed for the SLT department, provided by Medmark. Could anyone please share or scan it? I remember MMR, Varicella zoster, tb but I do not remember meningitis being mentionned. I do not remember the cost or the amount of doses given. Thanks in advance.
