Keep it REAL simple - have no areas that can be confusing.

Hours of employment: 9.00am – 5.30pm Break 11am @ 15 minutes, Lunch @ 30 minutes at anytime commencing from 12.30 to 1.30 subject to agreement.

    Dress code: Neat Casual.

    Holiday: 20 days per year

Sick Leave: The company does not provide payment for time off ill. In some circumstances time can be made up at a later date.

    Reporting to: Your name

Local regulation (applicable to all staff):

Food/Drink: Food / Drink cannot be consumed in the public area at any time.

Mobile Phones: The use of mobile phones during working hours is not permitted. Texting / answering and making of calls are restricted to designated break and lunch periods.

Yes it can be that simple - we had a NERA audit last year and at first he thought it was too basic, but then agreed it had everything required.

Miju's link is a very comprehensive contract, but more suited to a larger company.
