Aer Lingus Miscellanous Charges Order

Anyone used one of these before? It's a little book with a few pages After making a claim against Aer Lingus a year or so back,they sent me one of these.I now wish to use it for booking a flight. There's no voucher number on it though or anything. Any help greatly appreciated :)

I'd imagine you would need to call them - you wouldn't be able to book that online.
