You bought a van that, at the moment the tester inspected it, was fit to have a DOE cert issued. Ten minutes after he tested it, it could have carked it. A DOE test is a snapshot in time, not a warranty.

How much DOE was left on it when you bought it? If "recently" is within the last 3 months say, that means the van could have suffered any amound of abuse in the 9 months between the DOE and when you bought it.

Buy vehicles on the strenght of a proper pre purchase inspection, not a test you werent around for taken months earlier.

Run your van so its nearly empty of diesel then put about 60 euros worth of kerosene in along with about 500ml - a litre of 2 stroke oil then take it for a good hard drive, this will clean your injectors and fuel system out and you'll see a vast improvement in smoke ( lack of ) and also your vans performance.
I had a 03 transit which failed miserably on emmisions, I did the same as im advising you as was I advised by a very good mechanic friend of mine, took my van back for re test and she flew through.

ah, just trying to help as it worked for me and did the van no harm and id rather try a throwing 60 euro at it first before going straight in with hundreds, sure what harm can it do, worst case scenario is still needs new injectors and pump, best case, problem solved and he's not hundreds out of pocket. I know what id try first.
