Claimed back my 2011 health expenses last week, got my receipts from my doctor just incase I needed to produce them within the next 6 years, also have them for 2008, 2009 and 2010 just need to dig them out, I added small amounts to previous claims also within the last few days.

it says at the end of the letter if not sent on within 21 days it may result in a substantial tax liability, what does this mean ?

The amounts in question aint big.

What letter did you receive and what years does it cover? And are you PAYE only or self assessed?

PAYE, just health expenses it covers.

That is a standard letter you have received. Usually sent after a refund has already issued and would refer to paying back the refund. Interest and penalties are not really applied in PAYE. I would just send in the receipts as required within the timeframe given.

I added stuff to 2008, 2009 and 2010 small amounts after doctor confimed I can claim for this.

The '09 and '10 claims were process alright, P21 Balancing statements showed up on Wednesday.

I rang today asking when the amount would be in my account, they said there is a hold on it until I send my receipts in, is this normal even after p21 balancing statement is showing up ?

Yes. A certain percentage of all claims are double checked at various stages of the process.
