Increased Exemption Application Form

    I'm trying to locate the application form to claim the Increased Exemption of €10,000 applicable to a redundancy payment....I can't find on the revenue website. The Revenue leaflet IT21 refers to the application form but it doesn't seem to be part of the leaflet that I am looking at. Would appreciate assistance....

    Appendix 2 to IT21 on Revenue website. That's the application.
