I'm considering an extension and have been reading a few of the threads here.

When people talk about a price per sq ft depending on the quality of finish, what is typically included in the finish ? e.g. if it's a kitchen extension do the price estimates include the kitchen units, flooring etc or is the finish simply the quality and amount of lighting, sockets, windows etc?

more building contractors would price you for the structural and weathering work ie everything but decoration. This 'builders finish' tends to include one coat magnolia paint on walls, no floor finished etc.

Ok, so the equality of finish is down to quality of materials used ? If so, how does a lay man know if the builder is using quality materials.

the layman presumes its the bare minimum quality.. that's why you give the builder a drawings and a specification done by and arch/ arch tech/ interior designer.
