    The simplest thing is to get your self into an optician. They will have the correct form and be able to complete it.

    I printed the D.502 form in 2009 when applying for my LP (and I think it looked more like the first one), but when I went into the opticians, they had the forms anyway so I didn't actually need to print it off in the end.

    It's a very common request actually!

    Oh I was just wondering like doesnt it say on the form you can bring it to a doctor though, like could you actually do that?
    And also how long does it take to get the learners permit? (roughly)
