There is no 'rent agreement form' for use with the rent-a-room scheme. When you are still resident in the house- anyone taking a room under the rent-a-room scheme is living with you under licence and has no entitlements that a tenant would have. You have to be incredibly careful about going down the road of a formal agreement- as a lodger has no rights- but if you spell out rights and obligations on paper- you are giving him rights (and obligations) that he or she would not otherwise have.

When people use the rent-a-room scheme its more normal to do up a set of house rules without being too anal or else you'll never find someone to take the room. E.g.

A rota for buying cleaning supplies
Who has which cupboards and fridge arrangements
No loud tv or music after 10PM
No overnight stays without your prior consent
Turn off the immersion heater after use

etc etc etc- a set of groundrules for living with someone- not a rental agreement.........
