Illness benefit form question

    Just a quick question, I was on annual leave for the last few weeks after both paid and unpaid maternity leave, and am pregnant again. Due to complicatopns and a hospital stay last week I have been put on bed rest and have to be have a scan again next week to monitor the situation.

    There is a question on the date last worked on the form, is this the date last actually physically worked or the last day paid for work (annual leave in my case)

    Put down the date you last physically work, there should be a place at the end for notes you should include dates for Mat Ben, Annual Leave and/or Unpaid Leave. Or attach a note with these dates. AFAIK the date is to check if your claim links with any other claim or if the 3 waiting days apply.

    Thanks for that, just got them on the phone (had tried earlier before posting but to no avail)

    I was told to put down the last day of annual leave on the form.
