Help with Medical Card review form!

    I got this in the post last week. I am on JSA at the moment. The form requires evidence of my income and evidence of my PPS number. Can anyone tell me how I can fulfill these requirements? I get my JSA paid directly into my bank account and I've never got any receipt or proof. As regards the PPS number I haven't worked for a long time and only have a tax certificate from 2009 with my PPS number on it.

    Basically I'm wondering what everyone else here is doing? I'd really appreciate some help as I'm getting quite worried. I've tried ringing where I sign on but no one answers. Also the hatch I go to is only open Fridays 2-4.

    i got one today too. i have to renew my card yet again. this is going to be the 4th time

    i have no idea how to do it.. mummy will sort it out!: )

    Your bank statement could do as it has your PRSI number aand the amount you get paid on it.

    But they will probably want a letter form the dole office and a photocopy of your Social services card. You can call the tax office at any time and ask for a new tax cert they'll post it out to you.

    I actually requested a tax cert and bank statement today. I rang the SW office and they said there was nothing they could give me. They said the Medical Card crowd would have such information in their system already.

    Yes it is already on the system; my review forms have it half printed on.

    Even more interesting is that they have a new date of birth printed in the form; I have"lost" 40 years. lol ....
