Long Form Birth Cert needed??

    hi, I'm going to USA embassy to get my son's usa passport, it says i need his long form birth cert. Is this the one that was automatically issued to us when he was born? the one we have is A4 size, and contains both myself and my husbands names and ocupations and address?? It does not have any doctor's info, just the name of the lady that registered his birth.

    I'm confused as to weather it is his long form birth cert, or if i need to order another one??

    Any help much appreciated - thanks

    Long Form Certificates that we provide carry the official Government "HARP" symbol. Events registered after 2005 are now printed out in a new format which is a Light Green A4 Page with an image of a harp in the centre (as opposed to the older white version in landscape form).
