Help with CGT on Share Options - CG1 form

Would appreciate help from anyone with knowledge of CGT and form CG1. Apologies in advance for my lack of taxation knowledge.

Sold Share Options in July 2010 - Gain of €4237. Immediately sent RTS01 form with cheque for €1737 - 41%.

Sent my tax return for 2010 and should have been due tax back due to med1 expenses and other factors but they have ofset some of the rebate by a sum of €1186 as capital gains! This is the 1st time I've had to pay capital gains on share options? So I rang revenue and I think the issue is that they think I didn't sell the options immediatley when I excercised them. When I explained this, I was advised to fill out a CG1 form and declaire €0 gain.

Looking at the form, it's like double dutch so if anyone could tell me what boxes I need to fill out, I would be eternally grateful!
