New shoes...

Got a new pair of Shimano R133's on Saturday (and new cleats) and wore them for the Tour de Foothills.
At the 50k sandwich stop I noticed that my right foot was really painful just right of the ball of my foot, had to take off my shoe and massage some life back into it.. same when I finished the 100k.. I'm hoping its just new shoe syndrome, but could it be that I have my cleats set up wrong...?

Not sure, but I know all Shimano shoes that I tried on when recently buying were all of a very narrow fit for a given size. I'm presuming that you managed to give them a good try-out in a shop, rather than bought online?
Online shoe purchase is always a risk, but hopefully thats not the case for you..

My guess would be that your shoes were too tight. Thats assuming they are the correct size. It could be the cleats, but i would imagine that if your cleats were set up incorrectly, your knees would be giving you trouble by now.

If it was the sole of your foot it sounds more like a case of not being used to riding in super rigid carbon soled shoes which do take a bit of getting used to if you've been riding cheaper more flexy shoes until now. You could try a thicker insole for a while if you've got the space in the shoe. I think you'll find you'll get used to it soon enough anyway.
