Is an eyesight report needed for 2nd Provisional? Or is it only needed for the first one?

Also, do I need to get any photo's, or anything else signed by the Gardai? Seems I'll be applying for my 3rd license, so I'll be including the slip saying that I have tried to do a driving test, but unsure apart from that, the two photos, the fee, the application form itself, and the driving license, do I need anything else?

Meh. Applying for my 3rd Learner Permit, as my license says "992" (2nd Provisional).

Sweet. Will get the fee during lunch tomorrow. Question: what's the story regarding driving (with a fully licensed driver of course) when my license has been sent off to the County Council? Am I able to do so, or not?

Aye, have one of those slips saying I took and failed a test.
