    A member asked on my site regarding aircraft accident statistics caused by errors in maintenance. Here are 2 graphs I'd like to share here

    Are you getting that from "Other Human Error"? Note no figures were recorded in the 1950s, so the real figure is more 8-9%.

    Can't a number of mechanical failure issues be put down to things missed on inspection?

    Where does it say no figures recorded in the 1950's. There are figures recorded. The number 0 is a figure!! It states 0 not N/A

    And from a Quality point of view yes a number of mechanical failure issues can be put down to things missed on inspection?

    Could mechanical failure not also have an impact on this figure also??? Or am I reading this wrong? Wouldn't proper maintenence (in most cases) mean that mechanical failure is negligable?

    Alot would depend on the level of maintenance inspection required ie WEEKLY CHECK, A CHECK, C CHECK or D CHECK and what type of maintenance inspections are called up by the manufacturer

    There are no defined standards of Inspection level; different manufacturers will have applied their own standards and these need to be understood and published for the schedule/programme user's benefit.

    Examples of types of Inspection are:

    General Visual Inspection - A visual examination of an interior or exterior area, installation or assembly to detect obvious damage, failure or irregularity. Typically this level of inspection is made within touching distance unless otherwise specified. A mirror may be necessary in order to see all exposed surfaces and only available lighting, such as daylight or hangar lighting, may be necessary. Stands, ladders or platforms may be required to obtain adequate access.

    Detailed Inspection - An intensive examination of a specific item, installation or assembly to detect damage, failure or irregularity. Available lighting may be supplemented with a direct source at an appropriate intensity. Inspection aids such as mirrors and/or magnifiers may be required. Cleaning of areas may be necessary to carry out inspection.

    Special Detailed Inspection - An intensive examination of a specific item, installation, or assembly that may require specialised inspection techniques such as Nondestructive testing or boroscope inspection. In this case intensive cleaning may be required prior to inspection.

    Overhaul - the removal of a part, complete stripdown & inspection.

    Consider why SB's and AD;s are issued. When a none part suffers a failure in service and the manufacturer/authority want an immediate replacement or special inspection to prevent a reoccurance. Any part, even a new part could suffer failure before it reaches the time it is to be inspected
