    It's the most ambiguous, unclear antiquated form in existence.

    Some questions:

    a) Does the €15 fee have to a cheque?
    b) Where exactly do I send it to? (I'm in Clonskeagh, Dublin)
    c) How long does it generally take to receive it?

    a) You can use a postal order if you prefer
    b) the nearest office to Clonskeagh is the one in Nutgrove Shopping Centre
    c) took me a couple of days from the office in Smithfield.
