Swine flu and doctors cert / social welfare form required

    I was out from work for 3 days last week with suspected swine flu.
    I heeded the advice of the HSE and stayed in my house and treated myself instead of going to a doctor etc... as I'm not high risk and the symptoms didn't get too severe.
    I am back at work now, and work need a docs cert / social welfare claim form.
    So, do I have to go to the doc (now that I'm better!) just to get the form? Is there anywhere else I can get it?
    I don't think it's right I have to fork out money (and waste a docs time!) just to get a damn form.

    Any help would be appreciated.

    As far as I'm aware the social welfare form needs to be signed by a doctor.

    That much sick leave is always going to require a medical cert.

    the norm is that if u are out for 3 days plus you require a doctors cert, and the social welfare illness benefit is only payable after 3 days so you wont get anything from them.
