Help needed with Deed Poll

So, I know exactly what and where I have to do to get the Change of Name Deed Poll. What I cannot find is a Sample Deed poll that will be accepted in Ireland. As I do not live in Dublin, it would be very inconvenient to go down there with an unacceptable document ;D Also, if I am a UK Citizen, will I need to get a Letter…

It would be far easier to simply change your name by statutory declaration in the UK.
