Negative perfecionism

I know I'm pretty together and come from good stock but I also know I display negative perfectionism tendencies. At the worst times, they've been most notably, hyper judgmentality, low self worth and procrastination along with some relationship issues at times. These have been among the biggest traits of my behaviors over the years but I know I've made a lot of progress alone over the last few years.

I feel I have a lot of potential but I think that I can't release it. What could help me? Books? Professional help?

    Thank all.
