Potential problems renewing permit?

My first permit expired over a year ago, and has since been lost. I'm finally in a position where I can apply for a second one but have some problems.

If I got the first permit on the old test do I have to do the new one in order to get the second permit as it's been so long in between the two permits?

Also, I have no record of my previous permit's no. so who do I go to for more info about this/ will this cause major complications in applying for the new permit?
    Thanks

IIRC, you don't need to another test to get another permit, but you do need to go to your nearest gard station and have them stamp a form saying your first permit is lost(?)

No - you may allow up to 5 years between the expiry date of the previous LP before one has to go back to square one. As your 1st LP expired just over a year ago you may apply for a 2nd one.

I'm sure they'll work it out from your name, address, date of birth etc.

It is no longer valid, therefore there is no requirement to report it lost as it can't be replaced anyway.

I let my first provisional expire, then it got stolen with my wallet and I did nothing about replacing it. 6 and a half years later I applied again from scratch (under the learner's permit this time) and there were no problems. I did get a form signed by the Gardai and I can't remember what exactly I did in addition to that, but it was far from complicated.
