passport question

    I lost my passport a few weeks ago. I am applying for a new one but do I need a birth cert to go with the form. ????

    I don't know, but it should be listed on the form you're filling out ?

    It says if under 18 submit certificate. Then there is a lost passport section says nothing about submitting it.

    If you had a passport you could have just submitted that but in your case you are applying for a 2 years passport(then after 2 years you get a 10 yr) in case yours turns up within 2 years:

    You have to submit photo ID and a birth certificate because there is no other way to prove who you are otherwise without an old passport.

    Ring the passport office and get a definitive answer. They're very helpful over the phone.

    The Internet is great and all - but I wouldn't bet my time on it.
