P60 form needed

    Mods: please move if you feel this is the wrong forum but it's the best one I could think of.

    I've recently become the treasurer of a small primary school. We've a part time resource teacher who we pay directly (mainstream teachers are paid by the DES). This teacher never received a P60 for 2008. She only started in November so it's not a huge amount of money but she still needs to get a P60.

    However, I can't find a form anywhere. I've been on to 3 different departments in the Revenue and none of them have them to send me. Would anyone here know where to get either a hard or soft copy of a P60 form so I can give one to her?
