Approaching girls in pubs and clubs

Need some advice from the Ladies Lounge as I've no female friends I can ask this!

What's the best way for a guy to approach a girl and get talking to her in a pub/club?
Outside of cheesy chat-up lines (which I could never bring myself to use), what do lads say to you when they approach you?
Is it just a case of starting any old small-talk conversation? This is what I normally do when I make an effort but I'm often afraid that a girl will just find this boring! I do have a sense of humour but it doesn't always show until I get to know someone or conversation opens up.

Any genuine advice/examples would be appreciated. Thanks!!!

Don't be remotely drunk, and just make a general comment on the music and start a conversation on that, perhaps
I'd be far more likely to engage in a simple conversation than someone trying chatting up - that'd have me run a mile I think
