Kate was most likely on the phone to Cassidy, Sawyer's ex whom he had child with but he denied it was his. That's what Sawyer whispered to Kate on the helicopter before he jumped — he asked her to go find Cassidy and tell her he was sorry. As it turns out Kate already knew Cassidy. They met while Kate was a fugitive. I expect this will all be properly explained in a future Kate flashback.

Jack's downward spiral was already in progress when he heard Kate on the phone. It was motivated by his guilt over those left behind. Hurley kept telling him they had to go back, he was having visions of his father, etc but it took Locke's death to finally break him.
