Pretty much all you have to do is print off the ticket and show it at your desk whenever you're flying out. If you're flying to the states, make sure you get to the check in desk about 3 hours before you fly out. the queue at the visa control part (or whatever you call it) can be fairly lengthy, at least it was when we flew out last november.
i'm not really too sure how the whole filling out the visa form online works.

Most Airlines have people at the airport before you even get to the check in desk to check that you have all your particulars. A friend of mine working on behalf of Delta at check in said you would be surprised the amount of people who arrive and when ask about a visa they look confused and have never heard of it.

They book their flights online and just turn up like they are flying to someplace in the EU.

The staff will make sure you have everything you need. Prior to the US immigration even opening(Shannon for example as you are in Cork) the forms are sitting on the form desk. Grab a form and fill it in in advance of the desk opening. There are LED screens saying which flight is allowed to use the immigration. We were there recently and Continental and Us Airways were allowed up to the immigration before us and then they called all us Delta folk up. They were telling people to sit down and wait so dont fear you will know what to do when you get there.

As long as you have your ticket, passport and ESTA then you are just fine.

Make sure you have the return part of your ticket with you also otherwise you will denied entry into the US.

    Yes, you can.

No, they're not the same thing, so you need to provide both.

The ESTA reference number has nothing to do with the Advanced Passenger Information (API), as such. Aer Lingus won't ask you for the ESTA reference number :)
