Has anyone had problems on their trip and how much help (or how little) was your insurer.

I am booking travel insurance for a round the world trip have driven myself round the bend looking at all the policies- I have come to the conclusion that they would all pretty much suit my needs provided that if anything were to go wrong that they would pay out and not have me chasing after them for the next year. So my million dollar question is when push came to shove- how were the companies to deal with?
