A bank statement and a passport or drivers license will do the trick

As above drivers license or passport (picture ID) and a bank statment or a utilites bill (ESB, eircom, gas etc). (proof of address)

the proof of id must be in date and the proof of address (statement or whatever) must be dated in the past three months and for some networks, not in arrears
