The top sticker is the relevant one. It shows the camera body serial number (and lens serial number if it's a kit). Just enter the numbers on the claim form, print it off and put the sticker beside them on the form. There is no need to cut the box if you're careful peeling off the sticker.

I sent my claim on May 22nd and got my cheque on June 11th.

I cut the whole thing out to send them for my cheque, but I'm not sure which of the numbers I put on their form.

The form was a bit rubbish anyway from what I remember as it just tells you to print out the html form that you just filled in, in the browser, and send that to them kind of defeating the purpose of the fancy form filling in carry on prior to that.

If you put in all three of them, a note will come up underneath to say '40D kit' or something like that.

Just did my cashback on Sunday.

I put the serial from the body and the lens. Got my cheque today.

    hey, thanks all
i put the very first one in

dont know and some number are identical, i later noticed.

does one have to register for warranty?

Just check your body and your lens for the serial numbers, then attach the barcode that matches from the box (there may be some extra digits/letters on the ones on the box).

I only think you use the warrenty card if it breaks. Then you send the card and a receipt in with the lens/camera. But I am not 100% sure on that.
