support with cognitive impairment

    Hi,

    I was wondering who in the medical services ( if anybody) is responsible for assisting somebody with a long term physical illness that causes long phases of cognitive impairment in ensuring that they get proper medical care and make proper decisions regarding issues that crop up while very ill if there is no family support available? e.g. is this a role of a social worker or public health nurse or is the person just left to cope as best they can?
