    I've seen this screen before. But where does it come from? I'm guessing its a standard output from some type of http proxy, but haven't been able to identify which one.

    I've seen it on 3 (who are definitely behind at least one proxy) and now UTV is well.

    It's a webpage rendered by the transparent proxy, that you are behind. So it'll in your case be served by the proxies located at Three or UTV.
