hereby grant the (tick and fill in as appropriate)

The Photographer __ ____________________________________

and/or The Agency __ __________________________________

and/or The Client __ ____________________________________

and any licensees or asignees, the absolute right to use the photograph(s) and any other reproductions or adaptations, from the above Photographic Shoot.

I understand that the image shall be deemed to represent an imaginary person unless agreed otherwise, in writing, by my agent or myself.

I understand that I have no interest in the copyright, nor any moral rights in the photograph(s).

I am 18 years of age or older* __

I am under 18 years of age* __

*please tick as appropriate

Name of Model (PRINT): __________________________________

Signature of Model/Parent/Guardian: _______________________

Date: _____________________________

If under 18 years a Parent/Guardian must sign for the model.
