Six ft under series 5 dvd query

I missed the last five episodes of this and want to rent them to catch up. There are 5 discs in the set and twelve episodes so which disc do I need to start with? Have tried searching for a listing on the web to no avail.

Thanks for that. Those web rental places never tell you what's on each disc.
Funny that there never was a repeat of series 5 or another channel showing it. Thought more 4 were gonna do it but they stopped after the first series I think.
