Lost the census form

Returned late from the pub last night so went straight to bed with the intention of filling out the cesnus form before work. Spent 30 mins looking for the form this morning. cant find it. it could have been thrown out by accident.
Can i get a new one or anything????

last time one person got a short sentence and one person got a huge fine
the person who went to jail was making a protest about it being called a census of Ireland as the north is left out.
